Community service facilities for people with dementia in Nantou County
Target population:1. Individuals with suspected dementia.2. Individuals diagnosed and documented with a Clinical Dementia Rating (CDR) score of 0.5.3. Cases referred by long-term care management centers.Service items:1. Cognitive enhancement and dementia relief.2. Safety monitoring.3. Family support group (counseling).4. Family care courses.Service mode: Each service point shall provide at least 2 full days, 4 half days, or 1 full day plus 2 half days of service per week (minimum of 3 hours per half day).
