The form to be filled out: (1) 2 copies of the application form for the combined insurance of labor insurance, health insurance, and labor retirement (including basic information of the business unit). (2) 2 copies of the application form for adding insurance for labor insurance, health insurance, and labor retirement (including basic information of the insured workers). 2. Should be accompanied by a copy of the license: 2 copies of the front and back of the national ID card of the person in charge (employer) (for foreigners, please attach a copy of the passport or residence permit), and 2 copies of the relevant certification documents according to the unit category.
